Confirmed against reality recently enough to rely on.
Freshness is measured from the last confirmation, not the last edit — a page rewritten this morning by someone working from memory is not fresh. There is no button here to mark one verified: that claim means you went and looked at the live system, and a button on a list you are skimming would manufacture confirmations nobody performed.
Nothing has been confirmed yet. A page nobody has ever verified can never read fresh, however recently it was written.
